Please review the following checklist for all required underwriting information.

Submission Checklist:

Please note that all of the following is required prior to the release of a quote. It is the submitting agent’s responsibility to provide all of the information required, if not with the initial submission, shortly thereafter. Any delay in receiving this information will only delay the release of a quote.

The TCS application completed, signed and dated.

Four or five years detailed, company loss runs valued within 90 days for all coverages required (The insured must have a minimum of three (3) years motor carrier authority).

Details of all large losses over $50,000. For losses over $100,000, include:

  1. Loss circumstances
  2. Insured mitigation activities
  3. Potential for similar type losses
The insured’s most recent Financial Statement including Balance Sheet and Income Statement.
The most current vehicle schedule reflecting all vehicle types (Year, Make, Model, GVW and VIN#)
Vehicle inspection reports for all vehicles older than 15 years of age.
The most current IFTA, Schedule B, or company mileage reports.
A complete drivers list including Date of Birth, Date of Hire, Drivers License Number and Drivers License State.
MVRs on all drivers valued within 6 months.
A copy of the insured’s Driver Selection Criteria / Safety Manual.
A copy of the Trailer Interchange Agreement (If applicable).
A copy of your Non-Resident Brokers License.


All additional supplements must be completed and signed (If applicable).

Flatbed Supplement
Hazmat Supplement w/ a copy of the Authority


Completed and signed Supplemental Coverage Applications (If applicable).

Physical Damage Supplement
Business Auto Acord Application
Excess/Umbrella Acord Application
General Liability Acord Application
Motor Truck Cargo Supplemental Application
